If you’d like a copy of your medical records, contact our Medical Records Office at (209) 823-3111 or by fax (209) 823-1047. You will need a photo ID If you are picking up your medical records in person. If somebody else is picking up your records,
that person will also need a photo identification, as well as a signed authorization letter from you.
You can also request your medical records through My Health Rec, the hospital’s online tool for accessing
your most up-to-date information.
